Kirklees Young Persons Social Prescribing Service
Invictus Wellbeing is commissioned by South West Yorkshire Partnership NHS Foundation Trust to deliver the Kirklees Young Persons Social Prescribing Service across Kirklees.

The service provides crucial support to young people 11-25 with mild-moderate mental health concerns. We offer counselling, one-to-one support, signposting, groups, peer support and more through our central team in Huddersfield.​​​​​

How can Social Prescribing Support me?
Social Prescribing Support is focused on learning about what's importing you and using a non-medical approach to improve your health and wellbeing.

Once referred, one of our Youth Link Workers will get in touch to introduce themselves and understand your journey and what you'd like to work on together within the service.
​
It may be that you need some support around your mental health, you'd like to have counselling, want to get involved in a local sports club or simply just have a chat with someone who can understand. No matter what you come with, you'll find non-judgemental, safe and confidential support from one of our youth link workers.
What can you help with?
How does it work?
We support young people 11-17 and young adults 18-25 with a wide range of issues and concerns such as:
- Young people who are feeling low or a little anxious and want to chat with a trusted adult
- Young People who feel isolated and want to meet other people
- Young people who have been involved with CAMHS who would like to get some less formal support.
- Young people who need support and advice on practical issues like education, money, or their physical health.
- Young people who would like to improve their physical and emotional wellbeing.
How can I access support?
Simply make a referral using our referral form below. Once this has been completed, someone will be in touch with you within 5 working days to explore your support options. Then, you will be allocated to a relevant link worker. Our support typically lasts between 6 weeks and 6 months.
