What is it?
Invictus Wellbeing are commissioned by the South West Yorkshire Partnership NHS Foundation Trust to deliver the Open Minds Transition Support Service.
This service delivers wraparound mental health & wellbeing support to young people into less intensive and less formal levels of support.
How it works
Each young person is assigned a key worker who will support them on their journey from childhood/adolescence if they have struggled with mental health and into everyday life. This support can be long term and is built to improve mental health & wellbeing, resilience and everyday coping skills.
This support reflects what young people have told us they want in wraparound support and will include:
- Person Centred Counselling
- Therapeutically Informed Support
- Welfare Check ins/Appointments
- Peer Support Groups
- Workshops, Sports and Creative Activities
- Volunteering Opportunities & Social Action
- Social Prescribing & Active Signposting
How do young people access the support?
Appropriate Young people can be referred using the form below. If you are making a self-referral, we suggest speaking to us first.
